There is an administration charge of £20.00 per person for processing travel forms and to cover the increased cost of storing vaccines. Please ask a member of Reception staff for a Travel Pack and, once completed, return the forms together with the fee. We will then offer you an appointment with our Practice Nurse. Before your travel appointment you must complete a 'Travel Risk Assessment Form' (available from Reception) and return it to the Surgery a week before your appointment. This will enable the Practice Nurse to check your vaccination status and arrange to have any vaccinations you need ready for your appointment.
It is important to make your appointment for a minimum of 2 weeks before you travel. If you are travelling to more than one country then you must plan for your first appointment to be at least 6 weeks before you travel. If your travel plans are complex, you may need more than one appointment. The travel clinic is private ie not part of the NHS, the consultation is free but some vaccinations are chargeable (please see Travel Risk Assessment Form). We reserve the right to refuse appointments if we have limited availability as NHS work must take priority.
